Planet Oasis ("we", "us", or "our") is committed to protecting the privacy and personal information of our participants, their families, and visitors to our website (www.planetoasis.com.au). We are bound by the Privacy Act 1988 (Cth), the Australian Privacy Principles (APPs), and the strict confidentiality requirements of the NDIS Quality and Safeguards Commission.

We only collect personal information that is reasonably necessary to provide our Supported Independent Living (SIL) and Short Term Respite (STR) services. This may include:

  • Personal Details: Name, date of birth, address, email, and phone number.

  • NDIS Information: NDIS participant number, plan details, funding categories, and goals.

  • Sensitive (Health) Information: Medical history, disability information, Allied Health reports (e.g., Occupational Therapy assessments), medication requirements, and behaviour support plans.

  • Emergency Contacts: Details of parents, guardians, nominees, or Support Coordinators.

2. How We Collect Information

We collect information through lawful and fair means, primarily directly from you or your authorised representative (e.g., family member, guardian, or Support Coordinator). We also collect information when you submit forms on our website, email us, or speak with our team over the phone.

3. Why We Collect Your Information

We collect, hold, and use your personal and sensitive information to:

  • Assess your eligibility and suitability for our SIL and STR programs.

  • Develop and deliver safe, individualised, and neuro-affirming support services.

  • Communicate with you, your family, and your broader support team.

  • Process claims and manage billing through the National Disability Insurance Agency (NDIA).

  • Comply with our legal and regulatory obligations as a registered NDIS provider.

4. Disclosure of Your Information

We will not sell or rent your personal information to third parties. We may disclose your information to:

  • Our support workers and staff (only what is necessary for them to provide care).

  • The NDIA or the NDIS Quality and Safeguards Commission.

  • Medical professionals or emergency services in the event of a health emergency

  • Secure IT and software service providers who host our data within Australia.

5. Security and Storage

We take reasonable steps to protect your personal information from misuse, interference, loss, unauthorised access, modification, or disclosure. Physical records are stored in secure, locked cabinets, and digital records are protected by encryption, secure passwords, and restricted access controls.
